In a large skillet over medium high heat, brown the sausage, breaking it up with a wooden spoon. When the sausage is browned, remove it to a clean bowl. Drain grease and add olive oil to the pan. Saute onions and garlic until tender and fragrant. Add back in the browned sausage.
Pour in tomatoes and heavy cream. Bring to a boil and turn down heat. Simmer until sauce has thickened (15-17 minutes). Add drained pasta to the sauce and stir to combine.
Serve immediately with grated Parmesan and fresh basil if desired.
